Important points to be considered while filling up the SCOPE Exam form

1. Name of the Zonal Partner : Code No. will be 1/2/3 (Single Digit, It is Fixed for all centers and all districts)

2. Name of the Training Center & Address : Code No. Five digits (First two Digits are District Code, Plese refer the district Code list, last three digits are center code, please check the gmail group for your center code or call your Zonal Partner to confirm your center code)

6. & 7 : Examination Level Use the to tick the appropriate level.

8. SCOPE training center Code No. Six Digits (First Digit would be 1, second and third digit would be district code and last three digits would be Center Code
11. Application Form No. Check the form number from the top left hand corner of this form and please start marking the number from the reverse side.

12. If the student is physically handicap, please refer the table below
A Blind
B Deaf & Dumb
C Physically Handicap
D Miopsy
E Mentally Retarded

14. Student Photograph :  Ask for two photographs, Affix 1student photograph on the form and retain the second with you for the center.

17. Name of the Candidate: Fill in the candidate name and mark the OMR sheet.

18. Application Form No. Write the Application form No. as mentioned in point no. 11, do not mention the prefix 00, mention 49507 only

22. Examination Center Code: Do not fill this code
